This delightful 41 year old San Diego multiparous woman consulted us for revisionary breast surgery. She had previously undergone a bilateral breast augmentation many years previously with saline implants filled to 320 cc each. She now felt that her breasts were simply too large and heavy for her frame and lifestyle. She desired removal of her existing implants with a breast lift and a small increase in the breast volume with autogenous fat. She underwent removal of both intact saline implants, bilateral mastopexy (breast lift), and transfer of fat to both breasts obtained from her abdomen. She was delighted with the improved shape and diminished weight of her breasts and improved contour to her abdomen. She was able to exercise more easily and felt much more proportional in clothing. She returned to her usual activities within a few weeks following uneventful surgery.
